Alteration of Information in a Birth Register

Registration of a Past Birth Under Section 24

If the birth is over three months after the birth, but it has not been registered, you can submit the following documents to the Additional District Registrar and register your birth. 

• Request letter 
• Completed application form (Application form can be obtained from the Registrar Division of this office or any other Divisional Secretariat)
• Blank result sheet that the birth is not registered 
• Marriage certificate of parents of the child 
• Birth certificates of the eldest and youngest child 
• Written evidence of birthplace (Hospital birth report / Grama Niladhari report/estate birth report)
•  Health Growth Chart 
• School Admission Register / Student Task Index 
• Other birth certificates 
• Report of the Grama Niladhari 
• Parent's Birth and Personal Identity Card 
• A responsible affidavit stating that the date of birth is known.

Please contact the Registrar's Division for any correction of errors in the registration of Marriage (Kandyan) Certificates and Death Certificates.

2.Qualified persons for informing of death registration,

i.People who were there at the time of death
ii.An attendants at the time of death
iii.Medical Officer of the Hospital

3. Documents to be submitted

i. Duly completed statement
ii. Report from the hospital confirming the death

1. The death is registered by the Registrar of births and deaths of the area where the death occurred. The death occurred is registered by the Registrar of births and deaths of the area where the death occurred. The death occurred in the case of a private hospital, inform the Registrar of that area for registration of death.

1. The death is registered by the Registrar of births and deaths of the area where the death occurred. The death occurred is registered by the Registrar of births and deaths of the area where the death occurred. The death occurred in a rural hospital, inform the Registrar of that area for registration of death.
2.Qualified persons for informing of death registration,

i.People who were there at the time of death
ii.An attendants at the time of death
iii.Medical Officer of the Hospital

Documents to be submitted, 

i. Duly completed statement
ii. Report from the hospital confirming the death

Alteration of Information in a Death Register

The required declaration can be obtained from the Registrar Division of the Divisional Secretariat.
Submit the declaration to the Additional District Registrar of the Divisional Secretariat of the area where the death occurred.
The declaration should be submitted by one of the following persons:
-The person who provided the news to register the incident
-A trusted person who knows the truth of the matter
All information other than the date of registration in a death register may be amended.
The fee for the declaration is Rs. 50.00
Present written evidence to prove the authenticity of the request made in the declaration along with the declaration.

Obtain a Certified Copy of the Death Certificate.

An application can be obtained from the District Registrar's Division, Divisional Secretariat.
Charges
I. If you know the death certificate number and the registered date the fee is Rs. 100.00 per copy

II. If you do not know the death certificate number and the registered date, the fee for one copy is Rs. 200.00

III. Pay the required amount to the Registrar Division of the Divisional Secretariat.

Submit the completed application form to the Registrar.
If you like to obtain the death certificate by post, submit a stamped envelop with the application.
If the death is registered a death certificate will be sent and if the death is not registered a letter will be sent out informing it.

Dealing with the Kandyan Marriage and Divorce based on Application

You can apply for a divorce in an upcountry marriage based on one or more of the following:

• Adultery after marriage
• The man has committed adultery and his adultery, or cruelty
• To leave the husband for two years consecutively
• The husband shall leave the wife in full for two years
• Inability to live happily together.
