- If the registration number and the registration date of the relevant certificate are provided, Rs. 120.00 per copy
- If the registration number and the registered date of the relevant certificate are not given, Rs. 250.00 for the first copy and Rs. 120.00 for each copy of the certificate.

Registration of a Past Birth Under Section 24
If the birth is over three months after the birth, but it has not been registered, you can submit the following documents to the Additional District Registrar and register your birth.

Registration of deaths
Registration of a Death Occurred at Home
1.Inform the Grama Niladari within 07 days of the death occurs.
2.A report of the death by the Grama Niladhari will be sent to the Registrar of the area.
3.Death is registered by the Registrar of births and deaths of the area where the death occurred. Declaration of death can be obtained from the Registrar.
Qualified persons to inform
i) The nearest relative was present at the time of death
ii) A close relative who cared for the last sick
In the absence of such a relative,
The deceased is a relative of the deceased residing in the Registrar's Division
In the absence of such a relative,
i)A person who was present at the time of death
ii)The resident of the house where the death occurred
In the absence of the above persons
The person who buries, cremates or otherwise buries the body
Death can be registered within 03 months

Alteration of Information in a Death Register
The required declaration can be obtained from the Registrar Division of the Divisional Secretariat.
Submit the declaration to the Additional District Registrar of the Divisional Secretariat of the area where the death occurred.
The declaration should be submitted by one of the following persons:
-The person who provided the news to register the incident
-A trusted person who knows the truth of the matter
All information other than the date of registration in a death register may be amended.
The fee for the declaration is Rs. 50.00
Present written evidence to prove the authenticity of the request made in the declaration along with the declaration.
Obtain a Certified Copy of the Death Certificate.
An application can be obtained from the District Registrar's Division, Divisional Secretariat.
Charges
I. If you know the death certificate number and the registered date the fee is Rs. 100.00 per copy
II. If you do not know the death certificate number and the registered date, the fee for one copy is Rs. 200.00
III. Pay the required amount to the Registrar Division of the Divisional Secretariat.
Submit the completed application form to the Registrar.
If you like to obtain the death certificate by post, submit a stamped envelop with the application.
If the death is registered a death certificate will be sent and if the death is not registered a letter will be sent out informing it.

i.Residence of parties in the relevant division within 10 days preceding submission of the marriage notice. |

| 4. No party should have entered into any legal marriage which is valid at the time. |

If one party was not living in Sri Lanka within 10 days preceding submission of the marriage notice, the residence of the other party in Sri Lanka for such 10 days. iv. If none of the parties had not been living in Sri Lanka within 10 days preceding submission of the marriage notice, the residence of one party for 04 days in Sri Lanka. |

You can apply for a divorce in an upcountry marriage based on one or more of the following:

- From the Grama Niladhari of the applicant’s residential area;
- From the Estate superintendant (if the applicant resides in an estate);
- From the School Principal or the “Parivenadhipathi” of the respective Pirivena in the case of school applications
- Completed application along with the relevant documents should be handed over to the above respective officers as appropriate.
Necessary documents to be submitted to obtain an ID for the first time:
- Duly completed RPD Form 1
- Applicants under 50 years of age should compulsorily submit the Birth Certificate (BC) or the Probable Age Certificate issued by the Additional District Registrar in order to obtain an ID card.
- The Department considers 1st of July of the respective year as the date of birth in respect of the applicants who submit probable age certificates.
- Applicants over 50 years of age and do not possess the birth certificate or the probable age certificate, could submit two or more following alternative documents along with the results of a search of registers and an Affidavit to confirm the age.

- Applicants, who wish to include their occupation/designation in the ID card, should submit a service certificate obtained during the last three months. Professionals also need to submit documentary evidence, in case if they wish to include their profession in the ID card. (I.e. Engineers, Doctors, Accountants, Lawyers should submit relevant Degree certificates).
- In case of the Businessmen, they need to submit a certified copy of the Business Registration certificate.

- Buddhist cleric should submit the “Certificate of Ordain” or “Upasampada Certificate” issued by the Department of Buddhist Affairs
- “Dasasil Matha” should submit the registration certificate issued by the Department of Buddhist Affairs
- Islamic priest should submit the certificate issued by the Department of Islamic Religious Affairs
- Hindu priest should submit the certificate issued by the Department of Hindu Religious Affairs and Certificate obtained from the Department of Cultural Affairs
